The purpose of this program is to support innovative projects targeting the prevention and reduction of Type 2 diabetes and hypertension. Awards will be issued to community-based organizations, such as free health clinics, that provide services to vulnerable and underserved populations. Ultimately, this program aims to increase health equity and improve health outcomes among vulnerable populations utilizing evidence-based models of care. The program seeks to foster focused and measurable health improvements.
