Description:

Grants to Colorado agencies, congressional budget offices, foundations, health centers, districts, public health agencies, and dental clinics to provide dental care to underprivileged senior citizens over 60 years of age. Preference is given to applicants with the ability to serve a greater number of eligible seniors or will serve eligible seniors who reside in a geographic area designated as a Dental Health Professional Shortage Area. Preference will also be given to grant proposals that clearly demonstrate the applicant’s ability to outreach to and identify eligible seniors and to collaborate with community-based organizations.

Grant funding must be used to provide dental services to adults who are 60 years of age or older, who are economically disadvantaged, who are lawfully present in the state, who do not qualify for Health First Colorado or the Old Age Pension Health and Medical Care Program, and who do not have private dental coverage.

Qualified Grantees are required to:

  1. Identify and outreach to targeted eligible seniors and qualified providers;
  2. Demonstrate collaboration with community organizations;
  3. Ensure that eligible seniors receive covered dental care services efficiently without duplication of services;
  4. Maintain records of eligible seniors served, covered dental care services provided, and moneys spent for a minimum of six (6) years;
  5. Distribute grant funds to qualified providers in their service area or directly provided covered dental care services to eligible seniors in their service area; and
  6. Expend no more than seven percent (7%) of the grant amount for administrative purposes.

Eligible applicants:

Area Agencies on Aging, community-based organizations or foundations, Federally Qualified Health Centers, safety-net clinics, health districts, local public health agencies, and private dental practices are encouraged to apply.

Regional distribution of funds will be a key consideration in determining grant awards. If there is not sufficient funding to award grants to all eligible applicants, priority will be given to distributing the available funding across as many of the represented regions as possible. Potential grant applicants located in the following counties are encouraged to apply: Archuleta, Baca, Cheyenne, Clear Creek, Crowley, Custer, Delta, Dolores, Eagle, Garfield, Gilpin, Grand, Gunnison, Hinsdale, Huerfano, Jackson, Kiowa, La Plata, Las Animas, Mineral, Moffat, Montezuma, Ouray, Park, Pitkin, Prowers, Rio Blanco, Routt, Saguache, San Juan, and San Miguel.
