Description:
This funding opportunity will support safety net clinics in receiving technical assistance (TA) to improve their clinical and business capacity so they can offer high-quality, team-based comprehensive primary care. The Foundation will fund clinics up to $125,000 over the two-year period to support clinic staff participation in the funding opportunity and any education and travel-related expenses associated with implementing team-based care. TA for this funding opportunity will be provided by Nonprofit Finance Fund (NFF) and John Snow, Inc. (JSI).
Eligible applicants:
Be a nonprofit clinic or public agency that provides comprehensive primary care services to a significant population of patients who are living on low income and are uninsured and/or underinsured. For-profit clinics will also be eligible to apply for the funding opportunity, provided they meet the criteria and can also demonstrate:
- Urban clinics have an active caseload of at least 40% of Medicaid clients with a low profit margin
- Rural or frontier clinics have at least 30% Medicaid clients
