Description:
In light of the coronavirus (COVID-19) pandemic and its far-reaching impacts across the state, this funding opportunity has been modified to meet nonprofits where they are in this new landscape. This funding opportunity is designed to improve the capacity of clinics to serve more Coloradans with high-quality, comprehensive, and integrated primary care centered on patients’ preferences, needs and values. It will provide shovel-ready capital funding to safety net practices in Colorado in support of increased access to team-based, comprehensive primary care.
Proposed programs and/or projects must reflect the Foundation’s cornerstones, as their work is grounded in serving Coloradans who have low income and historically have had less power or privilege, putting the creation of health equity at the center of everything they do, and being informed by the community and those they exist to serve.
