Description:

The purpose of this program is to strengthen supportive services for adults and youth transitioning to adulthood with intellectual, developmental, or physical disabilities in order to maximize their ability to live independently and integrate into their communities. In addition, programs that support caregivers, particularly aging caregivers who are engaging in transition planning for the future care of their adult children with disabilities, will be supported.

Eligibility is limited to applicants serving the residents of Alaska, Arizona, California, Colorado, Hawaii, Idaho, Montana, Nevada, New Mexico, Oregon, Texas, Utah, Washington, or Wyoming. Refer to the Eligibility section for additional information.
