Description:
The purpose of this program is to increase access to coordinated, quality health care in a patient/family-centered medical home for children and youth with epilepsy (CYE) residing in rural and/or medically underserved areas, including racial-ethnic minority and low socioeconomic populations and populations with complex health needs and limited functional status. Funding will support quality improvement (QI) networks to address the content areas of increasing access to specialists through telehealth and telemedicine strategies; increasing family engagement at various levels across the health care system; improving the transition from pediatric to adult health care; and increasing communication, collaboration, and co-management between primary care providers and epilepsy specialty providers.
