Description:
The purpose of this program is to improve access to public transportation through building partnerships among health, transportation, and other service providers. Awards issued through this program will finance innovative capital projects for the transportation disadvantaged that will improve the coordination of transportation services and non-emergency medical transportation services.
The goals of this program are to:

- Improve health outcomes
- Reduce health care costs
