Description:
The purpose of this program is to increase access to health care and improve health outcomes for populations with high health care needs throughout the state. Funding will be allocated to rural, urban, and suburban areas throughout the state of Colorado, and will be focused on increasing access to direct behavioral health access. For the purposes of this program, behavioral health care is a continuum of services for individuals suffering from mental, behavioral, or addictive disorders. Services may be provided by clinical social workers, counselors, psychiatrists, psychologists, neurologists, peer groups, physicians, nurse practitioners, and physician assistants.
Projects must serve people who have an identified behavioral health issue and are a member of at least one of these populations that are underserved or unserved:
  1. Persons with multiple chronic or acute health conditions
  2. Persons impacted by racial health disparities
  3. Individuals enrolled in Health First Colorado
  4. Persons who lack health insurance coverage or have significant barriers to accessing coverage
  5. Persons who are experiencing homelessness
  6. Persons who are justice-involved
  7. Persons with a disability
  8. Persons whose primary language is not English
